Dean of Students - Transfer Reports

Greetings Counseling Discipline,

I hope you all are well!  As March 1^st and other transfer application deadlines approach, you may have students preparing their materials and inquiring about how to get their transfer reports completed.  If anything needs to be completed by the Office of the Dean of Student Affairs, please share this link with students - https://www.emailmeform.com/builder/emf/MCSA/DeansOfficeRelease.  We need this online form completed before we’ll release information.  Moreover, students can upload their documents using the form as well.  It is filtered appropriately based on the campus selected by the student.
